PROCESS PRIOR TO START OF RENOVATION

1.       Submit Electronic Application of Form # 1 to 3.

2.       Pay all the required bonds and dues.

3.       The contractor, to submit 3 hard copies of a signed Form # 3; (aside from the digital application).

4.       The contractor, to submit attachment i.e. plans (signed and sealed) as mentioned in Form # 4 to the admin office and to answer the Form # 4.1 (CoVid Questionnaire).

5.       When the plans are approved and the work permit is ready for issuance, the contractor must sign the Contractor’s Memo (Form # 5) at the admin office prior to issuance of work permit (Form #6).

 

DOCUMENTS AND OTHER ATTACHMENTS that is difficult to be scanned maybe submitted to the admin office such as: (BY CONTRACTOR)

1.       Building/Renovation Plan. (Signed and sealed)

2.       Detailed Scope of Work. (Signed and sealed)

3.       ID’s, PTR and NBI clearance.

4.       Other related documents mentioned in Form # 1 to 6.

Notice to Proceed. The Unit Owner allowing the contractor to start the work, to be submitted by Unit Owner. (NOT VALID WITHOUT APPROVED WORK PERMIT)

Work Permit will be issued after completion and evaluation of Form 01 to 05. This work permit will be submitted to the lobby guard three days prior to first delivery and prior to start of work, and must be at the door.
